To access your "Options" menu: Open the game on your device. In the top left hand corner of your game screen, next to your XP bar, you will see a tiny transparent list. Tap on the List. This opens up your "Options" menu. Go to the "Help" section and you will see your game ID number.
Use the "copy ID" button from that screen. This places the ID in your clipboard to paste into the form your filling.

Locate your player ID by tapping on the Cog Icon on the top right of your game.
You’ll notice the player ID beneath your username, once you've chosen yours.
To get your team name from Tap Sports Baseball:
Go to the settings menu
Your team name is located in the upper right corner of the screen
